The areas marked with an asterisk (*) are required to complete registration for membership in the Agora Survival Network (ASN). The information provided by every subscriber is protected from misuse, exploitation, or any unauthorized use by any source outside of the ASN system. All information provided by every subscriber is converted into coded identification information following a designated convention established by the ASN for specific use only by registered ASN members and the ASN as a whole. The registration questionnaire with all of the information on it will be maintained in a stand alone system that is not and will not ever be connected to any network either local, regional or world wide. Membership information may be used by the ASN system for the sole purposes of developing the network for the overall benefit of its members. Use of this convention by anyone outside of the ASN is strictly forbidden and is protected under US copyright / patent laws.

Enter the FIRST letter of your last name. (IE – your last name is Jones, you would select "J".)

Enter the LAST letter of your last name (IE – your last name is Jones, you would select "S".)

Enter the FIRST letter of your first name (IE – your first name is Dave, you would select "D".)

*Step 3 – Generating your Skill Set Designators (SSD’s). In order to complete this registration you must identify your personal Skill Set Designators (SSD) from the Skill Set Designator section. Please refer to this section and follow the instructions for that task. Once you have identified 3 personal SSD’s, return to this part of the registration and complete step 3 below.

First Skill Set Designator – this is the one that you want to be recognized for as being your most prominent skill.

Second Skill Set Designator – this is the skill that you would prefer to be recognized for as a backup to the first.

Third Skill Set Designator – this is the least favored skill that you possess.

Step 4: Your State of Residence

State where you reside – Using standard state two letter designator from the list at the end of this form, enter the designator for the state where you currently live.

Step 5: The city you live in, or are closest to

Please type in the closest city (greater than 200,000 population) to where you currently live.

Please indicate the (approximate) distance in miles from the center of that city that you live.

If you live within that city’s limit, enter 0 (zero).

Step 6: Request for assistance

Indicate if you have mobility issues and would like the assistance of fellow ASN members if you choose to move.

NO – I don’t wish to be assisted

Step 7: Donate to ASN?

YES – I am donating to the ASN

Since ASN does not ask for a photo for this profile or any other membership document, this is a voluntary opportunity to give us some personal details that may be useful to other members.
